Athletic/Activities Travel Release

Scarborough Athletic/Activities Policy is for student-athletes to take school provided transportaiton to and from sanctioned event when transportation is offered.

This form must be completed on an event by event basis and within 24 hours of the event in which you would like to transport your child.

In a special circumstance, if you need to drive your child TO an event please make that destinction in the question below. All request to transport a child TO an event will go through an approval process to be approved by the Department of Athletics.

Upon submittion of this form you will be brought to a confirmation page that includes the date and time you wish to bring your child FROM an event. You also will be sent a confirmation email. You and/or the guardian listed as the driver must show either the confirmation screen or the confirmation email to your childs coach before removing your child from the away event.

Students may not drive themselves. The parent/guardian listed as the driver must show either the confirmation screen or the confirmation email to your child's coach/advisor before removing your child from the away event. The parent/guardian completing this form must make sure the parent/guardian driver listed has the confirmation in order to show the coach/advisor.
Transportation Agreement
In lieu of the transportation provided by the Scarborough School Department, I give permission for my child to travel home with the parents/guardians listed above. II understand that Scarborough Public Schools asks that students ride the buses to and fromall athletic events and the departure from this requirement will release Scarborough Public Schools from liability for any adverseresults that may occur. The liability will be with the driver of the vehicle and the automobile insurance carrier. It is understood that the driver assumes all expenses related to transportation to/from a school sponsored activity.
